The Survival Tool You Need in Your Wallet or Shoe






Though we never want to be in a situation where we are incapacitated and cannot respond, unexpected accidents and sudden health emergencies can happen.
 
Should that ever happen, there are vital details that need to be passed on to emergency and medical personnel. Should no one nearby know you, this tool in your wallet could save your life. If you don't have your wallet with you, like when you're jogging, having this card on your person will help.

On a blank credit sized card (3.5 inches by 2 inches) add the following:

  • Emergency Medical Identification Card
  • First Name
  • Last Name
  • Birthdate
  • Address
  • Phone Number
  • Blood Type
  • Gender
  • Emergency Contact Name
  • Emergency Contact Relationship
  • Emergency Contact Phone Number
  • Family Doctor Name
  • Family Doctor Phone Number
  • Allergies
  • Other Medical Conditions
  • Medications
  • Date the card was created

I hope there is no need to ever use this card, but if there is, then the key people will have the necessary information they require at the right time. Have everybody in the family put one in their wallet.
